Can anyone advise on how often a silica gel pack should be changed in a safe or safety deposit box?

It depends of the humidity which can gon inside the box. Worst in summer, least in winter.
The best is having silica gel impregnated with an indicator: blue when dry and it turns pink when saturated with humidity. But if yours is colorless, impossible to say visually.
In this case, the best you can do is to dry them periodically (once a month for example). In any case, you can dry them in a kitchen oven set at 300°F and let dry for at least one hour (the packaging should resist to this temperature), or until the indicator revert back to blue. The silica gel can be regenerated indefinitely if kept clean.
