Here it is....the front was struck/clipped 3 times (once at top right, and twice on the lower left [across the D and the O of Dollar)

It was also clipped twice on the back along the center of the coin (you can see the matching clip lines along the bottom center rim of the coin)

Looks like as an after strike damage :(, Still inside the mint.

Maybe a jam in a sorter/counting/wrapping machine has caused this damage..
