It looks like the *D/O $1 note is scarce because a portion of the 80,000 printed were damaged.

Although the order for $1 replacement notes specified 64,000 notes to be numbered *A/A0468001 - 0532000 CBN delivered 64,000 notes numbered *D/O0468001 - 0532000. The order was, at least, 25% defective. Since the errors were scattered throughout the 2,000 sheets CBN agreed to replace the defective sheets and provide, without charge, a further 500 sheets of $1 replacement notes numbered *D/O0532001 - 0548000:
$1.00 2500 x 32/on = 80,000 notes numbered *D/O0468001 - 0548000"

CBN agreed to replace the defective sheets and provide, without charge, a further 500 sheets." This means that the damaged sheets from the first order were removed and replaced. On top of those now good 2000 sheets, a further 500 were provided without charge. Thus, the entire lot of 2500 sheets, as delivered (finally) to the Bank, were good notes.
