I guess this sheet would have been towrds the top of the stack based upon the serial number.  Are errors like this more likely to occur towards the top of the stack?

I would think that the sheet was folded as they were stacked, and yes it looks like it was closer to the top.  Many cutting errors are towards the bottom.  This was found in two separate bundles at a bank, maybe rejected by an instant teller.  It is great to have both pieces.
